Medicare CBD Initiative Could Expand Nationwide in 2027

The Trump administration has launched the promised initiative that gives some Medicare patients access to free CBD products. The people eligible to participate initially must be members of an accountable care organization (ACO), a type of healthcare network. The purpose of the program is to collect data to determine if CBD can improve the quality of life for older adults while potentially reducing healthcare costs.

CBD (cannabidiol) is nonintoxicating and derived from hemp, a type of cannabis plant. Products containing CBD are already widely used by older Americans. A 2024 study published in Clinical Gerontologist reported that over 14% of people aged 65 or older had used CBD products, including gummies, oils and tinctures, in the previous year. They are using CBD to address discomfort and sleep problems.

The goal is to collect real-world data about the use of cannabinoid products by older adults and whether the products may reduce their reliance on other medications or healthcare services. Sasha Kalcheff-Korn, executive director of Realm of Caring, said,

“Millions of older adults are already integrating cannabinoid products into their healthcare routines, yet the healthcare system has almost no infrastructure to understand what they are spending, why they are using these products, or whether these expenditures reduce other healthcare costs.”

Medical professionals have raised concerns about the initiative. Many seniors have medical conditions and take multiple medications. The concern is whether or not CBD will interact with the medications. However, doctors are willing to work with the initiative since their patients will collaborate with them on dosing and be monitored, another initiative goal.

Initially, five large ACOs are approved to participate, but the initiative will be expanded to include all 74 groups by January 2027. Patients in New York and Florida are the first to receive monitored CBD products.

This initiative is an overdue effort to modernize healthcare for seniors. It is already clear that they will use products that provide benefits, so creating a structured way to study how CBD actually affects them under medical supervision benefits them and, eventually, all CBD users. This initiative makes the use of CBD safer, may provide evidence that enables Medicare patients to take fewer medications, and closes a data gap.
